**This registration is for students ages Kindergarten-12th grade and adults 18 and older.

NOTE: By completing this registration, you are affirming the following: 
1) You have accepted Jesus as your personal Savior or you are a family member of someone Kindergarten  or older who has accepted Jesus as their Savior and are filling it out for them. 
2) You agree to allow Bay Hope Church to transmit images and names via the internet broadcast without compensation of any type. You agree to hold Bay Hope Church harmless of any and all liability associated with the transmission.
